"Am I in Perimenopause?" — The Symptom Checklist Your Doctor Probably Doesn't Have

If you’ve landed on this page because you typed something like “Am I in perimenopause?” or “perimenopause symptoms doctor won’t tell me” into Google—welcome. You’re in the right place.

At Luna Hormone Health in Massachusetts, we help women every day who are searching not just for information, but for a provider team who listens, investigates, and treats the whole picture.

Perimenopause can start earlier than most people expect—sometimes in your late 30s or early 40s—and it often shows up long before your periods start changing.

Two fun facts

1) Your perimenopausal symptoms can start up to 10 years before menopause

2) Period changes are usually LATE symptoms of perimenopause - not early.

Here’s the honest, approachable symptom checklist you wish you’d received at your doctor’s office.

What Exactly Is Perimenopause?

Perimenopause is the transitional phase before menopause, when hormones like estrogen and progesterone begin to fluctuate. These shifts affect everything from mood and sleep to metabolism and pelvic health.

Very consistently, women are told they’re “too young,” their labs are “normal,” or their symptoms are “just stress.”

Have you tried drinking water? What about a yoga class?

Not helpful.

If your body feels different and you’re trying to figure out why—this checklist is for you.

The Perimenopause Symptom Checklist

Every woman’s experience is unique, but these are the most common symptoms I see in my little Massachusetts clinic specializing in hormone replacement therapy, perimenopause, and menopause care...... (I've seen these a few times).

  • Worsening PMS or PMDD‑like symptoms — Mood swings that feel new or amplified.

  • Hot flashes or night sweats — Yes, these can begin years before menopause.

  • Sleep disturbances — Difficulty falling or staying asleep, especially around your cycle.

  • Anxiety or irritability — New or intensifying emotional changes without a clear cause.

  • Brain fog and memory lapses — Losing words or forgetting things more often.

  • Fatigue that isn’t fixed by rest

  • Waking up exhausted

  • Weight changes — Especially around the midsection, even with healthy habits.

  • Low libido or sexual discomfort

  • Breast tenderness — Particularly in the second half of your cycle.

  • Headaches or migraines — Often tied to hormonal fluctuations.

  • New joint aches or stiffness

  • Thinning hair or dry skin

  • Unpredictable or heavier periods — Even if they’re still “regular,” subtle shifts count.

Why Your Doctor Might Not Recognize These Symptoms

Most clinicians receive very little training (or zero) in perimenopause and menopause care. Many don’t connect symptoms to hormone shifts unless cycles are dramatically irregular.

And even then, traditional lab testing isn’t always sensitive enough to capture what you’re feeling day to day.

At Luna, we take a comprehensive, evidence‑based approach, combining symptom patterns, high‑quality hormone testing, and personalized HRT regimens. This approach provides us with a unique understanding of how perimenopause truly impacts an individual woman's health—physically, mentally, and emotionally.

When Should You Seek a Perimenopause Specialist?

If your symptoms are interfering with your quality of life, your confidence, your relationships, or your ability to function at your best—it’s absolutely time to talk to someone who understands perimenopause.

At Luna Hormone Health, new patients begin with a comprehensive package that includes visits, personalized care planning, and ongoing support.

Many continue with long‑term membership care because they love having a partner in their health journey—not rushed visits or quick dismissals.

Should You Consider Hormone Replacement Therapy?

If you are reading this, the answer is probably yes! Hormone replacement therapy (HRT) can be life‑changing when used safely and appropriately.

We also work with women who have more complex histories—such as breast cancer survivorship—where specialized training is essential. Even if you’ve been told HRT “isn’t for you,” it may be worth a conversation with a provider who has expertise in nuanced situations.

You deserve care that takes your symptoms seriously, treats you as a whole person, and helps you feel like yourself again. If you’re in Massachusetts or the Shrewsbury area and looking for perimenopause or menopause support, we would love to help you explore your options.
